You must not act as the collector for the employee being tested if you work for a _HHS-certified_ laboratory and could link the employee with a urine specimen, drug testing result, or laboratory report.
_"Problem"_ collection examples are situations like "shy bladder" and attempts to tamper with a specimen.
No less frequently than every _five_ years from the date on which you satisfactorily complete the requirements of Qualification Training and Initial Proficiency Demonstration, you must complete Refresher Training.
The _five_ mock collections (of Initial Proficiency Demonstration) must include two _uneventful_ collection scenarios, one _insufficient_ quantity of urine scenario, one _temperature_ out of range scenario, and one scenario in which the employee refuses to sign the CCF and initial the specimen bottle tamper-evident seal.
You must be knowledgeable about the current "DOT _Urine_ _Specimen_ _Collection_ _Procedures_ _Guidelines,"_ and DOT _agency_ regulations applicable to the employers for whom you perform collections.
You must meet the requirements of Basic Information and _Initial_ _Proficiency_ Demonstration before you begin to perform collector functions. (These are the first and third requirements).
You must demonstrate proficiency in collections by completing five consecutive _error-free_ mock collections.
If you make a mistake in the collection process that causes a test to be _cancelled_ (i.e., a fatal or uncorrected flaw), you must undergo _error_ correction training.
You must maintain _documentation_ showing that you currently meet all requirements. You must provide this on request to DOT agency representatives and to employers and C/TPAs who are using or negotiating to use your services.
As an employer, you must provide to collectors the name and telephone number of the appropriate _DER_ (and C/TPA, where applicable) to contact about any problems or issues that may arise during the testing process.
