Subpart D - W2 Collection Sites, Forms, Equipment and Supplies Used in DOT Urine Collections

As an employer, in Step 1-D of the CCF you may _preprint_ the box for the DOT Agency under whose _authority_ the test will occur.

As a collector, you may use a CCF with your name, address, telephone number, and fax number preprinted, but under _no_ circumstances may you _sign_ the form before the collection event.

When using an _electronic_ CCF, you must establish adequate confidentiality and security measures to ensure that confidential employee records are not available to _unauthorized_ persons.

As an employer, you may use an equivalent foreign-language version of the CCF _approved_ by ODAPC. You may use such a _non-English_ language form only in a situation where both the employee and collector understand and can use the form in that language.

As an employer, you may use an equivalent _foreign-language_ version of the CCF _approved_ by ODAPC.

As an employer, you are prohibited from using the CCF for _non-Federal_ urine collections. You are also prohibited from using _non-Federal_ forms for DOT urine collections. Doing either subjects you to enforcement action under DOT agency regulations.

The use of the non-Federal form is a _"correctable_ flaw." As an MRO, to _correct_ the problem you must follow the procedures.

What materials are used to collect urine specimens? For each DOT drug test, you must use a _collection_ _kit_ meeting the requirements.

You must use a _shipping_ _container_ that adequately protects the specimen bottles from shipment damage in the transport of specimens from the collection site to the laboratory.

You are not required to use a shipping container if a laboratory courier _hand-delivers_ the specimens from the collection site to the _laboratory._
